英利昔单抗

Riesgo muy bajo

Seguro. Compatible. Mínimo riesgo para la lactancia y el lactante.

Anticuerpo monoclonal IgG1 contra el factor de necrosis tumoral alfa (TNFα) usado en enfermedades autoinmunes como la enfermedad de Crohn, la colitis ulcerosa, la artritis reumatoide, la espondilitis anquilosante, la artritis psoriásica y la psoriasis. Administración intravenosa dosis inicial, a las 2 y 6 semanas y cada 8 semanas.

Sus datos farmacocinéticos (elevadísimo peso molecular y alta fijación a proteínas plasmáticas) explican la excreción en leche materna en cantidad indetectable (Mahadevan 2005, Vasiliauskas 2006, Stengel 2008, Kane 2009) o clínicamente insignificante. (Matro 2018, Glosen 2014, Fritzsche 2012, Ben-Horn 2011) 

Los niveles plasmáticos de lactantes amamantados por madres a las que se les administra Infliximab han sido indetectables o muy bajos. (Vasiliauskas 2006, Kane 2009, Fritzsche 2012, Steenholdt 2012)

No se han observado problemas a corto o largo plazo de desarrollo ni infecciosos o inmunológicos, ni problemas con la vacunación en lactantes de madres que toman Infliximab. (Mahadevan 2005, Vasiliauskas 2006, Stengel 2008, Kane 2009, Tursi 2010, Puig 2010, Correia 2010, Fritzsche 2012, Steenholdt 2012)

Su nula biodisponibilidad oral dificulta el paso a plasma del lactante a partir de la leche materna ingerida (Amin 2018, Bae 2012), salvo en prematuros y periodo neonatal inmediato, en los que puede haber mayor permeabilidad intestinal.

Numerosos expertos y sociedades científicas de Reumatología, Gastroenterología y Dermatología consideran de muy bajo riesgo y, por tanto, compatible su uso durante la lactancia. (Sammaritano 2020, Mahadevan 2019 y 2015, Anderson 2019, Picardo 2019, Matro 2018, Amin 2018, Briggs 2017, Nguyen 2016, Flint 2016, Gotestam 2016, van der Houde 2015, Nielsen 2014, Hyrich 2014, Mahadevan 2011)

Algunas agencias recomiendan no administrar vacunas de microorganismos vivos a lactantes mientras la madre esté recibiendo infliximab a no ser que los niveles séricos de infliximab en el lactante sean indetectables. (AEMPS 2022)
