Σιταλοπράμη

Riesgo muy bajo

Seguro. Compatible. Mínimo riesgo para la lactancia y el lactante.

Antidepresivo ISRS con acciones y usos similares a los de la fluoxetina. Administración oral en una dosis diaria.

Se excreta en leche materna en pequeña cantidad, aunque ocasionalmente con una dosis relativa superior al 10%. (Pogliani 2019, Schoretsanitis 2019, Weisskopf 2017, Rampono 2000, Schmidt 2000, Jensen 1997, Spigset 1997)

Los niveles en plasma de los lactantes cuyas madres lo toman son indetectables o muy bajos. (Berle 2004, Heikkinen 2002, Nordeng 2001, Rampono 2000).

Aunque hay algún caso aislado de alteraciones transitorias del sueño, irritabilidad o cólicos (Kronenfeld 2018, Schmidt 2000, Lee 2004), no se han observado problemas a corto ni a largo plazo en la mayoría de lactantes cuyas madres lo tomaban. (Werremeyer 2009, Berle 2004, Hendrick 2003, Rampono 2000, Jensen 1997)

Puede haber problemas de adaptación transitorios durante el periodo neonatal inmediato, como el síndrome de discontinuación de antidepresivos, en recién nacidos a término o prematuros que nacen con niveles plasmáticos altos porque sus madres toman citalopram durante el embarazo. (Erol 2017, Kieviet 2015, Franssen 2006, Nordeng 2001)

Puede producirse galactorrea asociada o no a hiperprolactinemia. (Türkoğlu 2016, Trenque 2011, Koch 2011, Wessels 2006, González 2001)

Diversas sociedades médicas y consensos de expertos consideran seguro el uso de esta medicación durante la lactancia. (Hale, Lactmed, Uguz 2021, Rubin 2004, Wisner 2002)

Son preferibles alternativas conocidas más seguras durante la lactancia, especialmente durante el periodo neonatal y en caso de prematuridad. (Weisskopf 2017, Sriraman 2015, Larsen 2015, Sachs 2013, Rowe 2013, Berle 2011, Patil 2011, Davanzo 2010, Lanza 2009, ABM 2008, Weissman 2004, Gjerdingen 2003, Nordeng 2001)

Las mujeres enfermas de depresión durante el embarazo necesitan más apoyo a la lactancia por tener más riesgo de problemas y cese precoz de la lactancia. (Grzeskowiak 2018, Leggett 2017, Venkatesh 2017, Gorman 2012)
