La mayoría de cefalosporinas se excretan en leche en cantidad no significativa desde el punto de vista terapéutico y no se han reportado complicaciones en los lactantes.
Las cefalosporinas son muy utilizadas en Pediatría con muy buena tolerancia, incluso desde el periodo neonatal, por lo que es muy improbable que, en pequeñas cantidades a través de la leche, puedan causar problemas al lactante.

Hay que tener en cuenta la posible negatividad de cultivos en lactantes febriles cuyas madres toman antibióticos, así como la posibilidad de gastroenteritis por alteración de la flora intestinal.
