Los probióticos son microorganismos vivos no patógenos que se administran con el fin pretendido de mejorar la salud. Administración oral. (Cucalón 2020, Gibson 2017, Williams2010)

A fecha de última revisión no hay pruebas científicas válidas de que el uso de probióticos sea eficaz para tratar mastitis o dolor mamario en las mujeres. (WHO 2022, Crepinsek 2020, Barker 2020, Paricio 2017, Espínola 2016, Amir 2016, Baeza 2015, Amir 2014)

Su uso indiscriminado puede retrasar otros tratamientos y ser oneroso económicamente. (WHO 2022, Amir 2016) 

Faltan pruebas de que administrados oralmente puedan llegar a la leche (Elias 2011).

El uso de probióticos pretendidamente preventivo durante el embarazo se asoció a un riesgo mayor de mastitis y otras complicaciones de la lactancia durante el primer mes de lactancia. (Karlsson 2019)

La administración de probióticos a la madre pueden modificar parcialmente la composición de la leche: se han visto cambios en el perfil lipídico, en los niveles de IgA (aumento o disminución, según trabajos) y en los de otros factores inmunológicos como citoquinas IL-27 y TGF-β1 y 2 (Kubota 2014, Nikniaz 2013, Luoto 2012, Hoppu 2012, Prescott 2008, Böttcher 2008). Sus efectos e importancia no son aún bien conocidos.

La investigación del uso de probióticos para tratar o prevenir varias condiciones patológicas, -enterocolitis necrotizante, asma, dermatitis atópica (Wickens 2013, Rautava 2012 y 2002, Boyle 2011, Foisy 2011, Dotterud 2010), cólico del lactante, mastitis (Fernández 2014, Arroyo 2010, Jiménez 2008), dolor mamario, vaginitis, gastroenteritis (véase ficha específica de Saccharomyces boulardii) y enfermedad inflamatoria intestinal, entre otras-, es muy prometedora, pero de momento presenta resultados muy modestos y con falta importante de trabajos científicos con calidad de evidencia de nivel 1. (WHO 2022, Crepinsek 2020, Barker 2020, Ehrhardt 2016, Foisy 2011, Thomas 2010, Boyle 2006)

Los probióticos parecen seguros (Snydman 2008, Borriello 2002), salvo en caso de déficit inmunológico, compromiso grave intestinal, diabetes o cardiopatía maternas y prematuridad (Sotoudegan 2019, Doron 2015, Boyle 2006). Aunque son raras, se han dado casos de infecciones graves por probióticos al administrar probióticos en pacientes con alguna de esas condiciones patológicas. (Poncelet 2021, Chakravarty 2019, Cavicchiolo 2019, Celis 2019, Martin 2017, Hayes 2016, Land 2005, Mackay 1999, Rautio 1999)

Las mezclas de probióticos (más que las cepas individuales) disminuyen el riesgo de enterocolitis necrotizante en prematuros. (Robertson 2019, Bührer 2019, Bi 2019)

La leche materna es una fuente importante de probióticos y prebióticos (Walker 2013, Weng 2013, Weber 2012, Novak 2001). Contiene numerosas especies de bacterias comensales (estafilococo epidermidis, estreptococo salivarius, enterococo, lactobacilos) con propiedades inmunomoduladoras, anti-inflamatorias e inhibidoras del crecimiento de bacterias patógenas como el estafilococo áureo (Heikkilä 2003), así como oligosacáridos no digeribles que favorecen el crecimiento de estas bacterias comensales en el intestino del lactante.

La lactancia juega un papel fundamental en la colonización del intestino del recién nacido (Walker 2013, Weber 2012, Martín 2012, Thum 2012, Penders 2006). Durante los primeros años de vida es diferente la flora intestinal de los lactantes según hayan sido o no amamantados.
