A beta-lactam antibiotic from the carbapenem group, for parenteral use only (IV/IM), indicated for moderate to severe infections (intra-abdominal, acute pelvic, community-acquired pneumonia, diabetic foot, and colorectal surgical prophylaxis).
It is excreted into breast milk in clinically insignificant amounts (Merck 2012, FDA 2001).
Authorised for paediatric use in infants from 3 months of age.
Its virtually zero oral bioavailability would make it unlikely for the drug to pass into the infant’s plasma from ingested breast milk, except in preterm infants and during the immediate neonatal period, when intestinal permeability may be higher.
Bear in mind the possibility of false-negative culture results in febrile infants whose mothers are taking antibiotics, as well as the possibility of gastroenteritis due to disruption of the gut microbiota (Benyamini 2005, Ito 1993, Kafetzis 1981).
