Certolizumab, certolizumab Pegol

Compatible

Producto seguro y / o la lactancia materna es la mejor opción.

Anticuerpo monoclonal humanizado contra el factor de necrosis tumoral alfa (TNFα) usado en enfermedades autoinmunes como artritis reumatoide, espondiloartritis axial, espondilits anquilosante, artritis psoriásica y enfermedad de Crohn con mala respuesta a terapia convencional. Administración inyectada subcutánea cada 2 semanas durante meses. 

No se han encontrado niveles detectables o muy bajos en leche materna (Morita 2018, Matro 2018, EMA 2018, Clowse 2017, Förger 2016, Mahadevan 2013) debido probablemente a su elevadísimo peso molecular. El polietilenglicol de su parte pegilada tampco se excreta en la leche materna. (Clowse 2017)

Por su naturaleza proteica es inactivado en el tracto gastrointestinal, no absorbiéndose, (biodisponibilidad oral prácticamente nula), lo que dificulta o impide el paso a plasma del lactante a partir de la leche materna ingerida. (Anderson 2018).

No se han observado problemas a corto o largo plazo de desarrollo ni infecciosos o inmunológicos, ni problemas con la vacunación en lactantes de madres que lo tomaban (Sahuquillo 2024, Membrive 2023, Lambiase 2022, Pieto 2021, Matro 2018, Clowse 2017, Mahadevan 2013 y 2012) y no se detectaron niveles, o muy bajos, en el plasma de dichos lactantes. (Mahadevan 2013)

Los fármacos anti-TNF reducen los niveles de factor de necrosis tumoral (TNF) en la leche materna (Sepiashvili 2025). Conviene vigilar la producción de leche ya que esta reducción del TNF, que interviene en la producción de leche, podría aumentar el riesgo de insuficiencia de lactancia. (Yan 2026)

El fabricante (EMA 2018) y autores expertos y asociaciones médicas lo consideran compatible con la lactancia.
