泼尼松龙

Riesgo muy bajo

Seguro. Compatible. Mínimo riesgo para la lactancia y el lactante.

La prednisolona es la forma activa de la prednisona. Su efecto es principalmente glucocorticoide.

Se excreta en leche materna en cantidades no significativas (Ryu 2018, Greenberger 1993, Ost 1985, Berlin 1979, Katz 1975, McKenzie 1975) y no se han observado problemas en lactantes de madres que la tomaban incluso por periodos prolongados de tiempo (Moretti 2003, Muñoz 2001, Nyberg 1998, Ito 1993), ni con dosis de 40 a 60 mg diarios. (Izumi 2014, Gan 2012, Westermann 2012)

En tratamientos crónicos o a dosis elevadas algunos autores recomiendan minimizar el paso a leche esperando 2 a 4 horas a amamantar tras la toma de la dosis. (Sammaritano 2020 y 2014, Götestam 2016, Damas 2015, van der Woude 2015 y 2010, Huang 2014, Bae 2012, Habal 2012, Van Assche 2010, Heetun 2007)

Grandes dosis intra-articulares de otros corticoides (metilprednisolona, triamcinolona) han afectado transitoriamente la producción de leche. (Babwah 2013, McGuire 2012)

Corticoides como la betametasona, administrados en el preparto, pueden provocar retraso en la lactogénesis II (subida de la leche) y disminuir la cantidad de leche en la primera semana. (Henderson 2008)

Los corticoides son de uso habitual en Pediatría y carecen de efectos secundarios cuando son utilizados aisladamente o en tratamientos cortos.

Diversas sociedades médicas, expertos y consensos de expertos consideran seguro el uso de prednisona/prednisolona durante la lactancia. (LactMed, Hale, Nguyen 2016, Götestam 2016, Flint 2016, Noviani 2016, Shah 2016, McConnell 2016, Bordini 2016, Huang 2016 y 2014, Briggs 2015, Schaefer 2015,Damas 2015, van der Woude 2015 y 2010, Durst 2015, Schulze 2014, Yarur 2013, Bae 2012, Habal 2012, Jain 2011, Van Assche 2010, Mottet 2009, Heetun 2007, Østensen 2007, N.Asthma 2004, Janssen 2000, Goldsmith 1989, Needs 1985)
