Μερκαπτοπουρίνη

Riesgo muy bajo

Seguro. Compatible. Mínimo riesgo para la lactancia y el lactante.

Antineoplásico antimetabolito e inmunosupresor. Interfiere la síntesis del ácido nucléico inhibiendo el metabolismo de las purinas.
Utilizado en el tratamiento de leucemias y de la enfermedad inflamatoria intestinal.
Es el metabolito activo de la azatioprina, que se convierte en el organismo en mercaptopurina.

Se han medido niveles bajísimos (Christensen 2008, Sau 2007, Moretti 2006, Coulam 1982) o indetectables (Ter Horst 2020, Kane 2004) en leche materna.
No se ha encontrado en suero de lactantes de madres tratadas con azatioprina (Zelinkova 2009, Sau 2007, Gardiner 2006) y estos lactantes no presentaron problemas infecciosos ni otros (Angelberger 2011, Zelinkova 2009, Sau 2007, Moretti 2006, Grekas 1984, Coulam 1982), incluyendo tres lactantes prematuros (Sau 2007).

Expertos y sociedades científicas consideran el uso de mercaptopurina o azatioprina de bajo riesgo durante la lactancia (Noviani 2016, Nguyen 2016, Mottet 2016, van der Woude 2015 y 2010, Huang 2014, Nielsen 2014, Yarur 2013, de Meij 2013, Habal 2012, Selinger 2012, Van Assche 2010, Gisbert 2010, Raj 2010, Christensen 2008, Mottet 2007).

Dada la fuerte evidencia que existen sobre los beneficios de la lactancia materna para el desarrollo de los bebés y la salud de las madres, conviene evaluar el riesgo-beneficio de cualquier tratamiento materno, incluida la quimioterapia, asesorando individualmente a cada madre que desee seguir con la lactancia (Koren 2013).
