Producto seguro y / o la lactancia materna es la mejor opción.

Macrólido inmunosupresor por inhibición de la calcineurina utilizado para evitar el rechazo de órganos transplantados (riñón, hígado, corazón, etc.) y en el tratamiento del eczema atópico, psoriasis y otras enfermedades autoinmunes (artritis, lupus, enfermedad inflamatoria intestinal...). Administración sistémica oral y tópica dermatológica.

Su elevado peso molecular y alta fijación a proteínas explican la excreción clínicamente insignificante obtenida en calostro y leche materna. (Kuczaj 2024, Akamine 2021, Hiramatsu 2018, Kociszewska 2018, Zheng 2013, Bramham 2013, Gardiner 2006, French 2003, Jain 1997)

No se han observado problemas de desarrollo ni inmunológicos, a corto o largo plazo, en más de 200 lactantes cuyas madres lo tomaban (Coscia 2025, Deans 2025, Kuczaj 2024, Mizutani 2023, Boyle2021, Klintmalm 2020,Nure 2019, Constantinescu 2014, Izumi 2014, Thiagarajan 2013, Gouraud 2012, French 2003), salvo un caso leve y autoresolutivo de trombocitosis transitoria. (Gouraud 2012)

Los niveles en plasma de dichos lactantes fueron indetectables o muy bajos. (Kuczaj 2024, Akamine 2021, Hiramatsu 2018, Izumi 2014 Bramham 2013, Gomez-Lobo 2012, Gouraud 2012)

Su baja biodisponibilidad oral, más disminuida aún en caso de coincidir con la toma de alimento (EMA 2015), en especial si este es graso, dificulta el paso a plasma del lactante a partir de la leche materna ingerida.

Los lactantes que han sido expuestos a inhibidores de la calcineurina durante el embarazo pueden tener deficiencia inmunitaria en el primer año de vida (Grimer 2007), de ahí la importancia de no ser privados de la lactancia materna sin un motivo demostrado.

El Tacrolimús puede provocar una leve disminución de los niveles séricos de prolactina, que no tiene repercusión en la producción de leche materna en una lactancia ya establecida. (Szypulska 2021)

Diversas sociedades médicas y consensos de expertos consideran seguro el uso de Tacrolimús durante la lactancia.
